The Program Coordinator will assist the House Manager with administrative functions including quality assurance, chart documentation, staff scheduling, maintaining the weekly activity/appointment schedule, assisting with client intakes, tracking staff certification, and ordering house supplies. The Program Coordinator also engages in case management activities with individuals including: intake, assessment, services plan development, linking and coordination of services, services advocacy, and monitoring and evaluation services. This position provides peer support services and consumer information for participants in the Outreach Program(s) as needed. The Peer Support functions as an integral part of the Outreach team and performs a wide range of tasks to assist participants in improving self-efficacy, identifying strengths and needs relative to achieving goals, and promotes empowerment and participation in the service planning process. The Peer Support provides support and outreach to homeless persons in a variety of settings. Outreach efforts are to identify and engage homeless and, at times, those at risk of homelessness as needed. These may be persons who are not interested or comfortable with traditional services or systems. The Assistant Director will directly support the Clinical Director of Supported Housing. This individual will provide administrative support to programs, including some data collection and analysis. The ideal candidate will have both strong clinical framework as well as be exceptionally organized. The Assistant Director will be completing assessment tools including ANSA, CSRs for program authorizations, and supporting treatment plan implementation. This person will work in all 5 of our current PNMIs at the direction of the Clinical Director.
